The Transition Coordinator RN is assigned to implement the standardized discharge program which is under the Case Management program in Medicine and Surgery. It's a full time Registered Nurse who works directly with case managers, with the Medicine/Surgery attending physicians, residents and interns, Nursing teams, Social Workers, Rehab teams, Community Living Center teams, Utilization Management team in Specialty and Hospital Based Service, transfer coordinators and all other members of the interdisciplinary teams. The incumbent is responsible for implementation of the standardized discharge program.
The major duties and responsibilities include, but are not limited to:
Educate patients about the diagnosis throughout the hospital stay.
Coordinate with the appointment coordinator to arrange for follow up and post discharge testing prior to discharge.
Responsible for delivery of quality care by using case management functions such as assessment, intervention, evaluation and safe discharge planning including appropriate documentation.
Responsible for the direct coordination of case management/CHNC assessment for the needs of patients for supplies and equipment.
Collaborates with all the interdisciplinary members of the team included physicians, nurses, social workers, Utilization Management team, Extended Care teams VA CLC staff, for program development, quality care, and resource planning.
